MMP Home Average Blood Pressure (BP) Submission

MMP Home Average Blood Pressure (BP) Submission

Please use a blood pressure machine which is validated by the British Hypertension Society – a list of these machines can be found on their website: http://bhsoc.org/

Use this monitor for a week, first thing in the morning and before going to bed at night. Each time, rest for five minutes in a seated position, your back well supported, your legs not crossed, and your arm resting on a flat surface with the upper arm at the level of your heart. Avoid caffeine, alcohol, smoking and exercise for at least 30 minutes before taking your blood pressure.

Take two readings at least one minute apart and record the lower of the two. At the end of the week, discard the first day’s readings, and calculate the average of the 12 readings recorded for your clinician.

Once you have done your BP twice a day for a week, please calculate your average (add the totals of the systolic numbers (top), then divide by 12; add the totals of the diasytolic numbers (bottom) BP then divide by 12) for the week and submit your reading to us.

What is your average BP reading

Please use this form if asked to submit your home average blood pressure (BP) value. The receptionist will forward this reading to the clinician, and you may get a call from our receptionist if there is a need for further discussion.

We would also recommend that you phone the receptionist a week after submitting this form, as the clinician will comment on the value.
