New Patient Registration

Our list is currently restricted for new patient registrations.  The receptionists will be happy to guide you through the procedure. When you do register, to speed up this process, we ask that:

    1. You live within our practice area (see map below)
    2. Please collect registrations forms from the practice (restricted to 4 registrations per day).  There are two forms that require to be completed.
    3. Please bring along with your two forms of identification, in person, to one of receptionists between 10am and 4pm.  One ID must be in photographic form i.e. passport, driving license; and the second must show a proof of address i.e. utility bill.
